How to Apply

Print and fill out the form above in entirety. Make sure to include your family size, preschool provider and proof of all applicable income (i.e. two most recent pay stubs, FIP or Child Support print out, last years Tax returns, self employment records or employer's statement of earnings). 

Once a family is approved, a contract will be sent in the mail to the family's home, the family is to sign and return the contract to our office. Once the signed contact is received by our office the preschool provider will be notified and arrangements for payment can be made with the provider and our office.
